Dr. Douglas Srygley with Austin Gastro discusses the advanced endoscopic procedure called ERCP (Endoscopic Retrograde Cholangio-Pancreatography). ERCP is both a diagnostic and therapeutic tool that enables your physician to examine the pancreatic and bile ducts. It is diagnostic in that it allows the physician to see the narrowing caused by the tumor in the pancreas and even take a biopsy of the tumor. ERCP is also therapeutic in that the physician can put a stint into the bile duct which drains the bile duct which will resolve the patient's jaundice, which will then help the patient with symptoms of jaundice such as itching and yellowing of the skin and eyes.

Gastroenterologist, Dr. Richard Sperling, explains Helicobackter Pylori (H. Pylori), how people become infected with this bacteria, how it is diagnosed, and how it is treated. H. Pylori infections can result in conditions such as chronic gastritis, peptic ulcer disease, gastric cancer, and lymphoma of the stomach. Common symptoms range from no symptoms at all to abdominal pain, bloating, nausea, lack of appetite, or bloody/black stool. There are invasive and non-invasive ways to diagnose H. Pylori. The non-invasive ways include H. Pylori breath test, stool samples, and blood test. Another way to diagnose H. Pylori is upper endoscopy with biopsy of the stomach lining to detect this bacteria. Treatment often includes multiple antibiotics and an acid blocker, followed up with testing to ensure that the treatment has effectively killed all of the H. Pylori bacteria. Reinfection can occur, but is rare once effectively treated.

Ulcers can be diagnosed by understanding the location, type, and duration of pain that is experience by the patient, and is also commonly diagnosed using upper endoscopy (EGD). Symptoms of ulcers are managed by the use of acid blockers and proton pump inhibitors.

Illinois Gastroenterology Group Joins GI Alliance
Dallas, TX – July 25, 2019 – GI Alliance (GIA), the nation’s largest independent gastroenterology services organization, today announced a partnership with Illinois Gastroenterology Group (IGG). With IGG and Texas Digestive Disease Consultants (TDDC) as partner practices, GIA now represents over 215 physicians in over 140 locations across three states. Terms of the transaction were not disclosed.
Serving the Chicago, IL metro area, IGG is the largest independent gastroenterology practice in Illinois and one of the ten largest in the U.S. by physician count. IGG was founded in 2010 through the affiliation of several leading GI groups and has invested significantly in clinical and administrative infrastructure as it has grown. In addition to core gastrointestinal (GI) services, IGG provides ancillary services including pathology, anesthesiology, infusion, and clinical research.
“This partnership is an important step in our evolution,” said Dr. Mitch Bernsen, President of IGG. GI Alliance’s unique model allows IGG to retain the identity we have painstakingly built over the years, while joining a larger team of physicians to pursue common goals and operational efficiencies, with a steadfast commitment to high-quality care for our patients.”

Waud Capital Partners (WCP), in partnership with the existing physician partners of GIA, invested equity capital in the transaction. GSO, the credit arm of The Blackstone Group, provided financing. Deloitte Corporate Finance acted as exclusive investment banking advisor to IGG.

About Dr. Lyles -
Dr. Lyles is a board-certified gastroenterologist at HEB Hospital in Bedford, TX. After graduating medical school, he completed both an internal medicine residency and GI fellowship at the University of Texas-Southwestern/Parkland in Dallas. During his fellowship, he had a special interest in inflammatory bowel disease (IBD) and trained in an IBD-specific clinic. Dr. Lyles has been practicing as a general gastroenterologist with Digestive Health Associates of Texas since July 2013.
He has worked as both a GI hospitalist and in a traditional office-based practice, which gives him a unique perspective in evaluating GI conditions. Dr. Lyles has been trained in upper endoscopy, colonoscopy, polyp removal, and treating gastrointestinal bleeding, in both the upper and lower GI tract. He has a special interest in inflammatory bowel disease (IBD) including ulcerative colitis and Crohn’s disease.
Dr. Lyles sees most of his patients through physician referrals from primary care physicians (PCP’s), internists, general surgeons, and OB/GYN’s. He is happy to see any patient suffering from a GI condition.

New Guidelines for Colon Cancer Screening
Austin Gastroenterology physicians, Dr. Navdeep Chehl and Dr. Shiv Desai, explain why everyone age 45+ should begin screening for colorectal cancer. The recommended age to begin screening was lowered to age 45 from age 50 because colon cancer has been occurring in younger and younger age groups. Colon cancer is one of the most preventable cancers, yet it is still 2nd leading cause of cancer deaths in the United States. A colonoscopy can detect and remove colon polyps BEFORE they have time to develop into colon cancer. Colon cancer prevention starts with screening.

Dr. Harry Thomas on the Long Term Prognosis and Treatment Plan for IBD Patients
Both Ulcerative Colitis and Crohn's Disease are chronic inflammatory bowel diseases (IBD) that involve inflammation of the digestive tract and can be quite painful, debilitating, and even life threatening for people suffering from IBD. Thankfully, these symptoms can be managed with regular ongoing treatment. There is no cure for IBD at this piont, but there is much research happening in this area and a cure is the goal. For now, most patients achieve and remain in remission, but ongoing professional evaluation and medical care is critical to manage any complications that may arise.

Dr. Adam Hafemeister of Austin Gastroenterology outlines some of the reasons why someone might need to have a colonoscopy prior to age 50, as recommended by the American College of Gastroenterology.
If a person has a family history of colon cancer, or even colon polyps, screening should typically begin at age 45. Other reasons a colonoscopy might be needed is when a person is experiencing an unexplained change in bowel habits (constipation and/or diarrhea), blood in the stool or black stool, abdominal pain & bloating, unintentional weight loss.

Dr. Harry Thomas of Austin Gastroenterology explains that the goals of a Low FODMAP diet are twofold. First and foremost we want to restore a person's digestive health and improve their quality of life. Many people wonder what happens if a polyp is found during their screening colonoscopy. Can it be removed right away? Would a second procedure have to be scheduled to remove it? Dr. Adam Hafemeister, with Austin Gastroenterology says yes, most of the time polyps can be removed during the initial screening colonoscopy. Only rarely, depending on the size and a few other situations, would a patient need to come back for a second colonoscopy to have the polyp removed. Remember, a colonoscopy is the only screening option that can both detect AND prevent colon cancer at the same time. Our goal is to catch & remove colon polyps before they have a chance to develop into cancer. Everyone 45+ should start a conversation with their physician about their family history of colon polyps and colon cancer to determine the age you should start screening. Most people with average risk should have their first screening at age 50.

Dr. Navdeep Chehl on KXAN: Avoiding the Painful Symptoms of Heartburn
Dr. Navdeep Chehl of Austin Gastroenterology stopped by KXAN's Studio 512 to give us some helpful tips on how to manage all of the family, food and fun over the holidays and bypass some of the painful heartburn and indigestion that can accompany all of the fun that comes along with the holiday season.
Some of the uncomfortable"Holiday Heartburn" symptoms include belching, chest pain, burning in the chest, nausea, and regurgitation, acid-reflux, and the feeling of food getting stuck after swallowing.
Common heartburn triggers include acidic and spicy foods, citrus and tomato-based foods, fried and fatty foods, mint-flavored candy, caffeine and alcohol.... basically all of the things that make the holidays so much fun!
The first thing to do to avoid holiday heartburn (or lessen the symptoms) is to enjoy all of these things in moderation, keep your body weigh in control, avoid smoking. In addition, a good strategy can be to wait a few h ours after eating to lie down to go to sleep, and sleeping on your left side has been shown to prevent reflux symptoms.
Two over the counter medications, Gaviscon and Pepcid, can be taken as needed before bedtime and they really help. However, proton pump inhibitors such as Nexium, Prilosec, and Prevacid - also available over the counter - aren't really appropriate for "as needed" therapy. These are better suited for regular, every day use, to be effective.

Britney Knowlton, a new mom and Physicians Assistant at Austin Gastroenterology, and Dr. Harry Thomas, a gastroenterologist with Austin Gastro, urge people to use caution when evaluating clinical information and interpretations from at-home DNA testing kits. Britney and her husband took these tests for fun as they began to plan to have a baby to learn more about their heritage. The portion of the test related to genetic diseases indicated that she had a high risk for Lynch Syndrome, an inherited condition that causes an increased risk for breast and colon cancer. Naturally, this caused a great deal of panic and concern for Britney and her husband. After seeing an oncologist for formal genetic testing Britney was relieved to find that she in fact does not have Lynch Syndrome. Dr. Harry Thomas explains that while these at-home DNA tests are useful for gaining insight into a person's ancestry, people should use caution when gleaning clinical information since these tests can be vulnerable to both false negatives and false positives, and that it is always best to talk with a doctor if you have concerns about your risk for genetic diseases.

Austin Gastro Beyond the Diagnosis: Living with Crohn's Disease - Erin's Story
Erin Bush has lived with Chron's Disease her entire life, but was not diagnosed until she was 18 years old when she happened to have emergency surgery because of severe abdominal pain. When she was formally diagnosed, Erin's doctors told her she would probably not be able to live a full life. They warned her that becoming a mother would be unlikely and that her quality of life would be severely impaired because of our Inflammatory Bowel Disease (IBD).
Erin's story is one of triumph over her diagnosis because she has realized so many of her dreams including becoming a mother, having a career, continuing her education, eating delicious healthy foods in restaurants, traveling and volunteering her time. The key to her success has been remaining steadfast with her treatment plans. She never stops taking her medications or infusion treatments, even when she's feeling great. Erin made it her mission to challenge the limitations her physicians laid out for her and worked with her doctors to find treatment plans that worked for her during various stages of her life. Erin is now in her 40s and hasn't had a single IBD flare in almost 7 years.

When it was time for Dr. Harish Gagneja's first colonoscopy upon his 50th birthday, he called upon his friend and business partner, Dr. Binh Pham to perform his procedure. Quite alarming statistics have recently come out that show colon cancer is being diagnosed much earlier than ever before. 1 in 5 newly diagnosed colon and rectal cancers are occurring between the ages of 20 to 54. The current guidelines recommend that people have their first screening at age 50 unless there is a family history of colon cancer, or a personal history of colon polyps. Anyone, at any age, who experience certain symptoms of colon cancer such as weight loss, anemia, blood in their stool, abdominal pain, and changes in their stool consistency should definitely talk with their doctor about their symptoms to determine if a colon screening would be a good idea. Most commonly, symptoms do not occur in the early stages of colon cancer, which is why regular screening and colon cancer awareness is so important.

Historically, colon cancer has been primarily diagnosed in people 50+. However, in the past 5 to 7 years there has been a 4-fold increase in the number of people in their twenties, thirties and forties being diagnosed with colon cancer. We aren't exactly sure why this is, but there is strong evidence suggesting a complex relationship between colon cancer and obesity, lack of exercise and poor diet. So it is possible that you could reduce your cancer risk by staying active and fit and eating a proper diet.

Dr. John Ziebrt of Austin Gastroenterology visited Studio 512 to talk about how to stay healthy while traveling abroad this summer. The best strategy to protect your gut health is to be proactive and focus on prevention. Take precaution with food and water in countries with limited resources. Avoid tap water, ice, mixed drinks. Use bottled water for brushing your teeth, and avoid things like salsa or fruit that can be left on tables. Hand sanitizer is always a good idea.
If you do become ill during your trip, most symptoms will resolve themselves withing 3 to 5 days without needing to see a doctor. However, if you have high fever, severe abdominal pain, bloody diarrhea, or symptoms lasting longer than 5 days you should absolutely seek medical treatment.

Dr. Pradeep Kumar Discusses HyGieacare Colonoscopy Preparation
Dr. Pradeep Kumar, a gastroenterologist with Austin Gastro, explains how HyGieacare, a same-day bowel prep, works as compared to a traditional bowel preps. Traditionally, before a person has a colonoscopy, they need to do a bowel prep which can mean drinking lots of fluid and running to the bathroom, sometimes disrupting a person's sleep. HyGieacare is an alternative that is done on the same day, just before the procedure, where the person's colon is completely cleaned out for them. Having a completely clean colon is critical to the gastroenterolgist being able to see polyps, lesions or any other abnormalities during a screening or diagnostic colonoscopy. Many people avoid or procrastinate their colonoscopy because they don't like drinking a lot of fluids, which is part of the traditional bowel prep, or they cannot afford to lose a good night's sleep. HyGieacare removes those barriers.

Dr. Chad Long Discussing New Hepatitis C Treatments
Dr. Chad Long with Austin Gastroenterology discusses how the treatments for Hep C have dramatically improved since 2014.
Prior to 2014 – treatments for Hepatitis C were so long, had so many side effects, and were relatively unsuccessful. Treatments were 6-12 months long and had a 50-70% chance of a cure rate.
Since 2014, there are new oral, anti-viral medications that are very well tolerated by patients and have a very high cure rate. Treatments are 8-12 weeks long with a 90+% cure rate in most patients.
Certain groups are at high risk for Hep C and need testing. These people include these people:
• History of IV drug use
• Born between 1945-1965
• Elevated liver function tests
• Received blood transfusion before 1992
Successful treatment of Hepatitis C can result in decreased need for liver transplant, a decreased rate of liver cancer, and decreased mortality overall in most patients.
Currently, Hepatitis C is very easy to treat and very the new anti-viral medications are very successful. All patients at risk for Hepatitis C should be tested and offered therapy.

Dysphagia is a difficulty in swallowing or abnormal swallowing. Difficulty swallowing means it takes more time and effort to move food or liquid you’re your mouth to your stomach. Sometimes pain is associated with dysphagia. Occasional difficulty swallowing may occur you’re your food isn’t chewed well enough or when you eat too fast, and this is not cause for concern. Persistent dysphagia beyond that is considered to be an alarm symptom and is something that should be evaluated by a gastroenterologist.
Dysphagia can be oropharyngeal in nature, which means there is difficulty initiating a swallow. Or it can be esophageal in origin which usually is a problem swallowing several seconds after the swallow is initiated.
Dysphagia can be caused by structural abnormalities such as rings, or webs, or esophageal cancer. Or it can be a motility dysfunction where the nerves and the muscles of the esophagus are not working properly.
This dysfunction in swallowing can be evaluated by:
• Radiology studies (barium swallow)
• Endoscopy
• Manomety
Treatments include surgery, endoscopy with dilation and multiple medications.

Inflammatory Bowel Disease is generally considered to be one of two things: Ulcerative Colitis which affects the colon, and Crohn’s Disease, which affects the entire digestive system including the colon. In general, people with Ulcerative Colitis will have diarrhea or pain in the left lower aspect of their abdomen. Crohn’s Disease generally affects the right side of the colon and the latter part of the small bowel.
Symptoms of IBS include:
• Migratory pain
• Cramping
• Discomfort
• Doesn’t affect people in the middle of the night
• Associated with stress
IBD treatments have made dramatic progress in the past 15-20 years. Today there are medical treatments that can help manage the symptoms of IBD without surgery and give people their lives back.

Colon cancer is the 2nd leading cancer in men and women and is an age-related disease. It is recommended that most people, with average risk factors, have their first colonoscopy at age 50 and repeat it once every ten years. African-American men and women are at higher risk for developing colon cancer and it is recommended that colonoscopies begin at age 45. Colon cancer is one of the most preventable cancers and colonoscopy is the gold standard for finding, and removing, pre-cancerous polyps before they have a chance to develop into colon cancer.

Diverticulosis is a very common conditions that affects approximately 80% of adults as they age, and occurs when small, bulging pouches develop in your digestive tract, most commonly in the colon. When one or more of these pouches become inflamed or infected, the condition is called diverticulitis. Symptoms include diarrhea, cramping and abdominal pain. If left untreated, diverticulitis can lead to potentially fatal infections and other serious health complications that require surgery.

Acute pancreatitis is a painful inflammation of the pancreas, located in the upper left side of your abdomen. There are direct and indirect causes of pancreatitis, including obstructions, immune system reactions, viral infections, and reactions to certain medications. Chronic pancreatitis is a long-standing inflammation of the pancreas that alters the organ's normal structure and functions. It can present as episodes of acute inflammation in a previously injured pancreas, or as chronic damage with persistent pain or malabsorption. Pancreatic cancer (ductal adenocarcinoma of the pancreas) in the early stages typically causes vague nonspecific symptoms. These symptoms and signs may include poor appetite, weight loss, abdominal or back pain, jaundice (yellowing of the eyes or skin, dark colored urine and/or light colored bowel movements) with or without itching, fatigue, and nausea.

GI doctors are frequently asked to evaluate patients complaining of chest pain. Aside from cardiac conditions are ruled out, there are other causes that are frequently the cause, such as musculoskeletal causes, gastrointestinal causes, pulmonary ideologies, and anxiety. Symptoms associated with non-cardiac chest pain include heartburn, trouble swallowing, and pain associated with eating. GI Diseases account for 10-15% of all instances of non-cardiac chest pain, so it is important that these patients are evaluated by a gastroenterologist. These GI conditions include GERD, esophagitis, motility disorders, and gall bladder disease. These conditions are most commonly diagnosed by utilizing upper endoscopy, esophageal motility testing, and imaging of the gall bladder.

One way physicians prioritize who should be seriously considered for a liver transplant is by looking at the patient's MESLD Score (Model End Stage Liver Disease). As of 2017, there are roughly 17,000 patients waiting for liver transplants, but only about 6,000 transplants are performed each year, making it all the more important to catch and reverse liver disease in its early stages.

Hepatologist, Dr. Nadege Gunn with Pinnacle Clinical and special consultant to Austin Gastro , explains Fatty Liver Disease, which affects 30% or more Americans due to the rise in obesity in this country. Obesity leads to fat in the liver, the fat causes inflammation which causes scarring, which leads to cirrhosis of the liver. If the associated complications commonly found with fatty liver disease are addressed, such as obesity, high blood pressure, high cholesterol, and diabetes are reversed, sometimes the cirrhosis can be reversed as well. However, if left unaddressed, that scarring (or cirrhosis) can become irreversible and can lead to significant health problems such as liver failure, liver transplant, liver cancer, and death.

Central to his practice is his dedication to evidence based treatments for gastroenterological diseases, establishment of outcome measures for ambulatory endoscopic procedures, and development of quality standards for ambulatory endoscopy centers. He has a number of published articles and a book chapter to his credit. As a member of the American College of Gastroenterology’s Practice Management Committee, his objective is to help his fellow members manage their practices. In 2012 he co-chaired the Practice Management Course at the ACG’s annual meeting. Dr. Morelli served on the QIQuIC Endoscopy Unit Quality Metrics Task Force participating in the formation of measurable quality standards for ambulatory endoscopy centers. Later in 2013 Dr. Morelli served on the Yale-CORE Technical Expert Panel assisting in the development of outcome measures for ambulatory endoscopy procedures.
For his work in clinical practice, research, service to the college, and GIQIiC, in 2012 Dr. Morelli was named Fellow of the American College of Gastroenterology. In 2013 he was named a top doctor in the city of Indianapolis.
Dr. Morelli maintains an active clinical practice with special interests in pancreato –biliary disorders, Clostridium difficile diarrhea, and colorectal cancers. He graduated from the University of Notre Dame in 1993. Then, he attended The Ohio State University College of Medicine. (EUS) Endoscopic Retrograde Cholangiopancreatography, or ERCP, is a specialized technique used to study the ducts of the gallbladder, pancreas, and liver. Ducts are drainage routes; the drainage channels from the liver are called bile or biliary ducts. If your doctor has recommended an ERCP, this page will give you a basic understanding of the procedure. If you have questions and concerns, please consult your doctor.
During ERCP, your doctor passes an endoscope through your mouth, esophagus, and stomach into the duodenum, which is the first part of your small intestine. An endoscope is a thin, flexible tube that lets your doctor see inside your bowels. After your doctor sees the common opening to ducts from the liver and pancreas, he passes a narrow plastic tube called a catheter through the endoscope and into the ducts. Your doctor injects a dye into the pancreatic or biliary ducts and takes X-rays with contrast.
How Should I Prepare for My ECRP?
Stop eating food and unapproved beverages by midnight the night before your endoscopy. You can drink clear liquids until five hours before to your scheduled endoscopy time. The following are approved clear liquids: water, apple juice, white grape juice, cranberry juice, broth, bouillon, tea, black or sweetened coffee, colas/sodas/carbonated beverages, Gatorade and other sports drinks, popsicles, and Jello.
Patients scheduled to go under general anesthesia, including Propofol, must stop clear liquids earlier. Please refer to the specific instructions provided by your physician.
What can I expect during ERCP?
Your doctor may apply a local anesthetic to your throat or give you a sedative to make you more comfortable. Some patients receive antibiotics before the procedure. You will lie on your left side on an X-ray table. As the endoscope passes through your mouth, esophagus, stomach, and duodenum, it does not interfere with breathing. However, you may feel a bloating sensation because of the air introduced through the instrument.
What are possible complications of ERCP?
Patients tolerate ERCP well when doctors who are specially trained and experienced in the technique perform the procedure. Although complications requiring hospitalization can occur, they are uncommon. Complications can include pancreatitis (an inflammation or infection of the pancreas), infections, bowel perforation, and bleeding. Some patients have adverse reactions to the sedative used. Sometimes the procedure cannot be completed due to technical issues.
Risks vary, depending on why the test is performed, what is found during the procedure, what therapeutic intervention is undertaken, and whether a patient has major medical problems. Patients undergoing therapeutic ERCP for stone removal face a higher risk of complications than patients undergoing diagnostic ERCP. Your doctor will discuss your likelihood of complications before you undergo the test.
What can I expect after ERCP?
If you have ERCP as an outpatient, you will be observed for complications until most of the effects of the medications have worn off. You might experience bloating or pass gas because of the air introduced during the examination. You can resume your usual diet unless you are instructed otherwise.
Someone must accompany you home from the procedure because of the sedatives used during the examination. Even if you feel alert after the procedure, the sedatives can affect your judgment and reflexes for the rest of the day.

Fecal Microbiota Transplant (FMT) is a procedure in which fecal matter, or stool, is collected from a tested donor, mixed with a saline or other solution, strained, and placed in a patient, by colonoscopy, endoscopy, sigmoidoscopy, or enema.
The purpose of fecal transplant is to replace good bacteria that has been killed or suppressed, usually by the use of antibiotics, causing bad bacteria, specifically Clostridium difficile, or C. diff., to over-populate the colon. This infection causes a condition called C. diff. colitis, resulting in often debilitating, sometimes fatal diarrhea
C. diff. is a very serious infection, and the incidence is on the rise throughout the world. The CDC reports that approximately 347,000 people in the U.S. alone were diagnosed with this infection in 2012. Of those, at least 14,000 died. Some estimates place that number in the 30,000 to 50,000 range, if the U.S. used the same cause of death reporting methods as most of the rest of the world.
Fecal transplant has also had promising results with many other digestive or auto-immune diseases, including Irritable Bowel Syndrome, Crohn’s Disease, and Ulcerative Colitis. It has also been used around the world to treat other conditions, although more research in other areas is needed.
Fecal transplant was first documented in 4th century China, known as “yellow soup”.
It has been used for over 100 years in veterinary medicine, and has been used regularly for decades in many countries as the first line of defense, or treatment of choice, for C. diff. It is customary in many areas of the world for a newborn infant to receive a tiny amount of the mother’s stool by mouth, thought to provide immediate population of good bacteria in the baby’s colon, thereby jump-starting the baby’s immune system.
Fecal transplant has been used in the U.S., sporadically since the 1950's, without much regulation. It has gained popularity in the U.S. in the past few years, although experts estimate that total number of treatments to date in the U.S. remains below 500 patients.
In late spring of 2013, the FDA announced it was classifying fecal matter as both an Investigational New Drug (IND) and a Biologic, and that only physicians currently in possession of an approved IND application would be allowed to continue performing fecal transplant.
This resulted in less than 20 physicians in the U.S. being allowed to perform fecal transplant. There was a groundswell of opposition from physicians and patients, and on June 17th, 2013, the FDA reversed their position, and announced that qualified physicians could continue to perform FMT for recurrent C. diff. only, with signed consents from patients and tested donor stool.
This has resulted in more and more physicians beginning to perform fecal transplant, but there are still only limited numbers serving the large population needing the treatment. There are also many patients who do not have a donor to assist them.
And there are many patients who have never even heard of this treatment, even though the success rate for treatment of recurrent C. diff. is estimated to be well over 90%.
In all documentation, dating back to 4th century China, there has never been a single , serious side effect reported from fecal transplant.
Fecal Transplant is a low-cost, low-risk, highly effective treatment. It is not currently covered by most insurance companies, as it is still classified as an experimental treatment.
