"My personnel online file had a lot of erroneous data that I did not enter. I corrected some of the info but could not correct it all. I gave the nurse a list of the medication I take but to my knowledge it was not corrected before I left. It said I have/had colon cancer which is incorrect. I would like to see a copy of the corrects I told the nurse about. This should have been done before I left there. Also, a person requesting data over the phone needs to be able to speak English clearly so you can understand what they are asking you."
"I have used Digestive Health Associates for over 33 years. The doctor my husband and I was using is retired now, but he saved my husband life by being a very through and caring Doctor. After his retirement I chose to use Dr. Lyles and have not regretted that decision. He has been my doctor now for 10 years. He is very through and I respect that.
