"My personnel online file had a lot of erroneous data that I did not enter. I corrected some of the info but could not correct it all. I gave the nurse a list of the medication I take but to my knowledge it was not corrected before I left. It said I have/had colon cancer which is incorrect. I would like to see a copy of the corrects I told the nurse about. This should have been done before I left there. Also, a person requesting data over the phone needs to be able to speak English clearly so you can understand what they are asking you."

After his retirement I chose to use Dr. Lyles and have not regretted that decision. He has been my doctor now for 10 years. He is very through and I respect that.
